What is HealthWorks?

HealthWorks of Illinois is a collaborative effort of the Department of Children and Family Services, Department of Healthcare and Family Services and Department of Public Health.  The primary purpose of HealthWorks is to assure that DCFS wards from birth to age 21 who are in substitute care, receive comprehensive quality health care services.  By linking each child to a primary care physician the child will receive higher quality and more consistent care, as well as reduce dependence on emergency room care. Logan County Department of Public Health serves as the Lead Agency for thirteen counties:

Lead Agency

The Lead Agency responsibilities include maintaining a physician network of primary care providers and specialty care providers in all ten counties specifically for foster children.  As well as the development, implementation and management of the network of HealthWorks providers and services.   

Medical Case Management

The Lead Agency is responsible for coordinating Medical Case Management activities with local health departments.  Every child, birth through age 5, has a medical case manager located at their local health department.  The Medical Case Manager assumes the responsibility to arrange, support and facilitate DCFS wards access to and follow-up with primary health care, specialty, sub-specialty and related services in order to ensure that wards receive medically necessary comprehensive health care. 

HealthWorks was developed to provide the best possible medical care for each DCFS ward, including linkage to a primary care provider and ongoing monitoring of their case throughout their stay in the DCFS system.
